When filling out your enrollment form you will need to check the % of gross wages option...  And check the weekly option  for the payroll frequency. And you will need to enter what percentage you choose in the box, (5, 7, 10 or 13 for Journeymen and 5 for Apprentices) Then proceed to the confirmation page. Once your contribution election is complete, a confirmation page will be emailed to you. You must provide a copy of this confirmation to your employer to have your contribution deductions begin. ​
